Travel Note
CC: Patient is a 24 year old Caucasian male who presents with a chief complaint of “being madly in love.” HPI: Patient reports first experiencing these symptoms approximately 18 months ago after encountering Valeda Yong at a baseball game. He reports that when he met her, he considered her “attractive” and “interesting” but wasn’t really expecting anything profound to come of it. However, after spending more and more time with the object of his affection, patient reports that he began to “fall in love,” with symptoms including planning a future around her, marked positive change in mood when she is present, and changing behaviors to make her happy. (He cites watching “the Bachelor” as an example of the latter.) At present, our patient considers himself “hopelessly in love” with Valeda Yong and has no desire to change his current state.

PMH: 1. Patient reports that he has “maybe” been in love before, but “never like this.” 2. Patient reports a distant history of asthma. 3. Remainder of history not pertinent to case. PSH 1. I mean I guess the patient got some surgery on his finger once but this isn’t really pertinent to the case. Social History: Patient currently lives with a roommate in an old 1920s house, but reports extreme excitement to move in with his girlfriend. It is suspected that this action will only increase his symptoms of being “madly in love” with Valeda. - Patient is an “occasional” drinker, reports that he often consumes “more than he means to” because his girlfriend “makes him finish her drinks and that actually means like finishing half the drink.” - Patient does not smoke, drink, or use other drugs

ROS: Neuro: Patient reports occasional headaches from “dealing with Valeda’s test anxiety” but reports that they are “worth it” in the context of everything else. CV: Patient reports an increase in heart rate whenever Valeda enters the room. Says this is “probably good for my heart health.” No chest pain, change in exercise tolerance, edema. Resp: Patient reports occasional difficulty breathing when Valeda decides to lay flat on him, but refuses to complain about it. GU: No complaints, except for a recent urge to inform people when he’s using the toilet. Psych: Patient reports altered decision making, as his priorities have shifted from being egocentric to focusing on what will make Valeda happy. Patient’s taste in TV and entertainment has shifted, as he has found himself very reluctantly enjoying the Bachelor and Botched, along with other seriously trashy television. (The guy has started following along with the vampire diaries. I think he needs help.) Physical Exam: Unremarkable.

Assessment 1. Patient is an otherwise healthy 24 year old Caucasian male who presented with a chief “complaint” of being “Madly in Love” with Valeda Yong. Although it has caused significant changes in his behavior and priorities, he has no desire to change it. 2. Patient is exhibiting signs of altered mental status, as he has begun to watch the Grammys, Vampire diaries, the Bachelor, and other television he never previously enjoyed. This appears to be an unfortunate side effect of his extended close proximity with Valeda Yong. When asked, patient reports that it is “disconcerting but worth it.”
Travel Note
Plan 1. In order to balance out his recent consumption of trashy television, I highly recommend that the patient begin consuming quality television such as “Battlestar Galactica” and “Black Mirror.” This should somewhat offset the cultural damage inflicted by “The Bachelor” and “The Vampire Diaries.” 2. Patient has no desire to alter his state of being madly in love, as he’s as happy as he has ever been. Thanks for a great 18 months <3. Here’s to many many more! Love, Scott
