A rental car gives you flexibility to: explore beaches, visit Old San Juan, see El Yunque National Forest, grab groceries/snacks, and move around easily. If you plan to: mostly stay at the resort, relax poolside, and enjoy wedding festivities...you can definitely have a great trip with airport transportation and Uber.

Puerto Rico is tropical, so mosquitoes definitely exist. The good news: major tourist/resort areas actively manage mosquitoes. Zika is no longer considered a major outbreak issue and most travelers simply use standard bug spray precautions.
