We are providing COVID-19 testing at NO charge to you.
Please RSVP if you will be attending our testing site ASAP in order to provide an accurate count of tests to the clinic. Please note: All you will need is your ID and insurance card ( if you do not have your insurance card,we will provide a form to fill out) Due to county/state guidelines: Our venue is requiring we provide proof of Full Vaccination OR a NEGATIVE Covid-19 test result at the Venue entrance. Please take a moment to answer the question below as it will help us stay organized and keep record when providing our venue with a final count. If you have any questions please reach out. This is a black-tie event, men please wear a tuxedo, formal suit and tie. Ladies please wear a formal dress.
We kindly ask you arrive at least 15-30 minutes early in order to check in and have our ceremony start on time

We are having an unplugged ceremony, we kindly ask you refrain from using your personal cameras and cell phones for videos and pictures.
