It is not required unless you are unvaccinated. Though a negative COVID test is required within 24 hours of your departure from Costa Rica. If you test positive, you will need to quarantine for 10 days. Travel insurance would pay for any medical expenses and accommodations during that time. Our wedding planner recommended using Trawick International. Yes, US cash is accepted country-wide, so there is no need to exchange to colones (unless you would like to of course!). Bring $20 bills or smaller (larger denominations are harder to spend because there are problems with counterfeit $50s and $100s, though most restaurants do tend to accept $50's). Also make sure they are in good condition with no tears. Change is given back in colones.
No, unless you plan to do day trips and want to explore further than just the town of Manuel Antonio! In MA the roads are narrow and there is little parking. There are taxis that run all night and an excellent bus system during the day (every 20 minutes until 7pm).
