How do I get base access?

Download the file and fill out all of the highlighted information, including both social security number AND driver's license. Under sponsor, please put Claire Blume and phone number (847) 915-2213. Please submit the pdf files to Claire's secure Dropbox: https://www.dropbox.com/request/HNJDdXnz8bPp1yg6h14u This form needs to be filled out for all guests over 18 who do not already have base access (i.e. active duty, retiree, dependent, or government contractor ID).
