In accordance with the City of Chicago's Health Order No. 2021-2 (Proof of Vaccination in Public Places), effective January 3, 2022, Revolution Brewing will require that all guests entering the premises show proof that they are fully vaccinated against Covid-19. Details of the health order are attached. Revolution is requesting that guests provide this information before our event. You will need to fill out a Google Form with your contact information (for contact tracing purposes) and upload proof of vaccination (photo or scan of vax card, screenshot of digital record from vaccine provider/healthcare app, etc). You must be signed into your Google account to fill out the form. THE GOOGLE FORM LINK WAS SENT VIA EMAIL. For folks who don't have Google accounts or who do not wish to fill out the form, you will need to email the information required directly to the event planner, Stephanie Aksland at steph@revbrew.com. Please provide your first and last name, email address, phone number, and proof of vaccination. When you arrive at Revolution, please state that you are here for the LaManna-Szajowski Wedding, and you'll be directed to where you can check in. The Revolution event staff will have a master list of who RSVP'd and provided their proof of vaccination info. Revolution will need to verify your identity, so please be prepared to show a valid ID (driver's license, state ID, passport, etc.) Parents can verify for children under 16. *Please also be prepared to show your proof of vaccination again just in case. While they are making every effort to create an efficient system, they are also adapting to these very new changes. Your patience and understanding is greatly appreciated! Please also note that masks are still required in Chicago and the state of Illinois, regardless of vaccination status. Please wear a mask when entering/leaving the building or event space, when going up to food stations and/or the bar, and when closely interacting with Revolution staff.
