Please fill out and return your RSVP Card with this information on the back as soon as possible. (If you have already sent back your RSVP card please email this information to -------@gmail.com) On the back of your RSVP card for each guest attending please write: -Name -Date of Birth -Email address -Phone Number As per NYS guidelines, The Piermont will notify the Suffolk County Health Department of our wedding and will maintain a record of contact tracing information for 28 days after the event.

All attendees must receive a NEGATIVE diagnostic test result for COVID-19 OR proof of completion of the COVID-19 vaccination series at least 14 days prior to the date of the event. PCR: Negative Result within 72 hours of the event start time. (Therefore, your PCR test must be taken after 6pm on 5/5) Rapid: Negative Result within 6 hours of the event start time. (Therefore, your rapid must be taken on 5/8 after 12 pm.) All guests must give proof of their negative test result (paper form) to event staff immediately upon arrival to the Piermont. OR COVID Vaccine: proof of completed COVID-19 vaccination at least 14 days prior to the date of the event. (Vaccine completed by 4/24.) ***PRINTED Proof of vaccination or Negative Test will be required upon arrival to the venue.*** Make sure you bring a paper copy of your negative result or completed vaccine as you will give it to security and not get it back. You cannot show a picture on your phone. It must be a printed copy!
