Example - Suzie and Shirley are bridesmaids rooming together and each are going to pay half the cost of the room.  Suzie will complete a form and list Shirley as the second traveler on her form, and Shirley will complete a form on her own and list Suzie as the second traveler.  Another example - Jack and Jill are a married couple.  Jill takes care of all the travel arrangements and payments for them so she'll fill out a form with both her and Jack's information and send it in.
