Amid the ever-changing rules & regulations associated with the on-going pandemic, we have included additional questions as part of our RSVP that will greatly help us best plan for June 26th. As part of the Illinois Phase 4 mitigations, individuals with proof of full vaccination or a negative COVID-19 test (PCR) 1-3 days prior to an event or outing do not count against capacity limits. We understand that these are strange times, but appreciate your feedback to help us ensure a comfortable experience for everyone to celebrate with us! For question 1 or 2 (as applicable), please respond with the Name & "Yes" or "No" for all individuals included on your invitation. (Example for Question # 1: John - Yes, Jane - Yes, John Jr. - No)
