The University Club of San Francisco is requiring all guests and vendors to provide (1) either proof of vaccination or a negative covid test within 72 hours prior to the event and (2) a signed COVID Liability Release Waiver.
For proof of vaccination, any of these options will work:
1. Physical vaccination card
2. Photo of your vaccination card
3. Digital vaccine record from https://myvaccinerecord.cdph.ca.gov

With regards to the use of masks, they will be optional once cocktail hour starts.
