There are only 4 US cities that have direct flights into Cartagena (CTG airport) Miami (MIA), Ft. Lauderdale (FLL), Orlando (MCO) and New York (JFK). Due to this you are likely to have a connection if you are not flying out of these cities. Once in Cartagena, the airport is about 10-15 minutes away from any point in the main city.
